The error means that there is something wrong with input parameters in your “Add attachment” action.
I have made a test on my side and don’t have the issue that you mentioned. The flow works successfully as below:
Please check if you fill proper File Name and File Content in “Add attachment” action.
Besides, please check if you have set Has Attachment and Include Attachments property to YES. The screenshot of trigger’s configuration as below:

I am facing similar issue.
I am unable to get the attachments to SharePoint list item, please see below screenshots:
Can you please help me with where i went wrong or why is this issue happening which is stopping attachments to go into list ite.
FYI: this flow is creating succesfully a list item but just attachments are not coming in same.
I am able to attach all the files from Emails to SharePoint list item for most of the file types. For .txt file type, If I provide filename manually with .txt extension, it works otherwise it throws an error. Any ideas on how to resolve it?

I followed the flow described here, but instead of using email as trigger, i used Flow button to trigger the flow.
When the button is pressed, it will as for a file name and file to be uploaded to an SP list.
And i'm able to update the sharepoint item. However, I attached a PDF file, but when i opened the item it just shows the file without any file type.
Is there an extra step i need to do to keep the same file type when I update an item with an attachment?
