Approval flow doesnt work, i dont know why.

My flow looks like this:

  1. When a new email arrives (V2)
    1. has attachment
    2. include attachment
  2. create item (sharepoint)
  3. apply to each (attachment)
    1. condition -- Attachments ends with pdf or docx
      1. if yes:
        1. apply to each (attachments):
          1. Add attachment (Sharepoint)
        2. Start and wait for an approval
        3. Condition 2
          1. If yes: send email (v2) " the thing has been approved "
          2. if no: send email (v2) " the thing has not been approved "
      2. if no:
        1. delete item (Sharepoint)
        2. send an email (v2) " the thing requires attachements! "

if I test it....

i keep getting the wrong approval responses and the errors are:
