Approvals return null reponses despite succeeding

I have a power automate where the Start and wait for approval return null responses.

What is the cause of this?

It seems that our users have been using ms teams to approve approvals and some of the users clicked the cancel button. I was wondering how can we remove the cancel button in teams.
