Building form to planner flow

Hi,

I'm trying to build my first flow and continue to get an error with the most basic update task action.

I would like to pull in several fields from the form into the description, but I can't even pull one without getting an error.

What am I doing wrong?

@rleb

Can you post a screen shot of your Flow in edit mode?  Also, please post a screen shot of a run which shows where the Flow generated the error.  Without that, it will be difficult to assist.
