Case closure details and survey

Hi Team,
I have configured the flow send survey when case resolved in CRM using email template. But we need mention case resolved details and survey in email template. Kindly suggest your comments.

Thank you for response. I have found the alternative way below screenshot. One more query customer,resolve by field not fetching from the CDS. Kindly suggest your points.
