I did a MSFT Forms and created 2 sharepoint lists.
One SP list is called Greater China & Korea and the other one is called ASEAN.
One of the questions on the form is if you are in ASEAN or Greater China & Korea.
What I want to do have in the flow is that if the person selects ASEAN in the forms for example, the answers will got tot the ASEAN sharepoint list only.
The problem I have is that all the answers are going to both sharepoint lists.

You should add Condition below Get response details action to check which list the user has selected.
Then configure the two Create item actions in the two branches of the Condition.
Please try again.
First of all I would get rid of the 'Apply to each' and move 'Get response details' just after your trigger.
Now, you need to add a 'Condition' action block just after 'Get response details', evaluate response to your target answer there
'Condition' action block will set up two branches: true (yes) branch and false (no) branch.
So assuming you compare the response to your target question with value 'ASEAN', in your true branch you need to add 'Create item' in the ASEAN SP List; in your false branch you need to add 'Create item' in the alternate SP list

I created the condition but didn't work as planned.
Basically you have 4 possible paths, so I did 4 conditions:
My flow is:
First (1) if Business Unit is equal to Greater China & Korea then create the item in that Sharepoint List.
If no, move to second condition (2): if Business Unit is equal to ASEAN then create the item in the ASEAN Sharepoint list.
If no, move to third condition (3): if Business Unit is equal to India & South West Asia then create the item in the India & South West Asia Sharepoint list.
If no, move to final condition (4): if Business Unit is equal to Japan then create the item in the Japan Sharepoint list.
Your screenshot is so big I just can see the boxes, but not the inputs assigned 😞

May I suggest you to add a 'Compose' action block just after 'get response datails', assign as its input 'Business Unit' response, reexecute the flow and share its contents?
Since you did considered suppressing 'Apply to each' yet, please also share the nr of times 'Apply to each' is executed also
I tried with the compose, but the item was not added on the Sharepoint list of Japan. It was added on Greatet China and Asean, despite selecting in my form only Japan.

All the inputs are false in the conditions, but the condition 4 should be true since it is for Japan.
You are right, it had an enter. It was now added in Japan SP List 🙂
The problem is that it was added to China and ASEAN too, have a look:
So, the flow added the item to condition 1, 2 and 4. Despite being true only 4.

Partially solved haha, the item still appeared on the SP list of China and ASEAN. Should appear only in Japana.

Please check again your last flow execution... How many times is your 'Apply to each' being executed?
If more than once, please remove the 'Apply to each'

What caught my attention is that the first and second conditions, even though were false and the create item shows not satisfied. , an item was added to China and Asean:
I added a project called "Flownaut" for India & South West Asia
But it was added to SP list of China, ASEAN too 😕
To the Japan SP it wasn't added which is correct. The problem seems to be with China and Asean SP lists.

Aaaargh! I am afraid I cannot import it, it crashes... I guess due to the fact I do not have permissions to access the Forms Survey.
Can you share several screenshots from your current flow design? I mean you already did it but in a single image that was so big I could not see the inputs assigned in each step
I think the problem was that I had activated the old flow haha. So I just deactivated it.
