Ok, it looks like there is somehting wrong with the format of the excel file.
Can you try a create table in flow so that Flow creates your excel file in one drive? Then compare the format of the file.

I'm afraid that it's the dynamic content of the trigger "Powerapps" in the "Insert row" action that cause the flow to run failed.
Could you please take a try to insert some custom value instead of dynamic content of the trigger "Powerapps" inside every field of the "Insert row" action to see if the issue still exists.
Please let me know if the error still exists.
