Create File Creating Corrupted Word File

I have a pretty straight forward flow that takes a user input from a PowerApp, Get Items, and then uses those items to fill out a word template.  Problem is the word docs the flow is creating are coming out corrupted.  It worked for a few tests, and now has stopped working.  When I try to open the doc from SPO or Word desktop, it can't open the file.  I tried converting the document to .zip file to see what I was getting and the resulting file is not a valid archive.  I added the delays because I thought maybe it was going too quick.

Hi @csteiner73 ,

It seems that the second Apply to each Action is unnecessary. In addition, the Dynamic content (PO#) can be replaced by an expression.

I have made a simple test for your reference.

1. Here is my SharePoint List.

2. Here is my Flow.

outputs('Get_items')?['body/value'][0]?['InternalColumnName']

In addition, here are some links for your reference.

Getting rid of the second "Apply to Each" and cleaning up the values fixed my issues.  Thanks a ton for your help!
