I have a multiple choice question on a form where the answers are yes and no, I want to create a task in planner if the person clicks the yes choice and submits the form. This is the flow I tried but it does not work.

Could you please check if there is any blank after the "Yes"? If there is blank after it, the output of the answer will be "Yes "(There is a blank):
It will cause the condition will never met:

What do you mean by "not working"? Is the flow failing for any error, or is it because of the condition (it always go to the "no" branch) ?

It is not failing with any error it is just not creating the task. Assuming its because of the condition, its always going to the no branch.

Did you try the following:
Enter details in the following fields to show up the task:

b. Assigned User Ids - some default users.
After providing the details, the task will show up for the users.
Otherwise, the task will stay as orphan in the plan but you see the task under Group by Labels view.

Yes, I tried this and it still does not create a task in the bucket.

Did you check Group by Labels view in the planner by logging in with the same user whom the task is assigned.

Could you provide a screenshot of your execution results?
All I need it to do is create a task in the correct bucket if the person that submits the form checks the "yes" key. Here is a screenshot of the form, the flow I used and the bucket I need it to create the task in.
@Anonymous, can you check "Group by Assigned to" or other views?
I think, the task may be creating but not assigned appropriate metadata to it. Due to that, it is not showing you at correct place.
