Creating Office Forms using Power Automate with data from Dataverse and storing the response having files as well into dataverse

Hi,

We have a requirement where a scheduled Flow needs to be created and a mail needs to be sent to few team members with a questionnaire link (Microsoft Form) having dynamic values coming from dataverse, and response with files needs to be recorded in Dataverse.

These are the requirements:

1. MS-form needs to be created dynamically through the same scheduled flow as the form will have few dynamic details like incident numbers etc. to be picked up from Dataverse.

2. A mail needs to be triggered to users with the form link

3. Responses of the form needs to be saved to Dataverse along with the files uploaded through the form.

Please let me know in steps how to implement this.

Yes, the form creation makes it difficult. As I see it, you have a few options:

  • Create a form using the Power Automate "When a HTTP Request is received" trigger. You will be able to expose Dataverse data (and be able to upload files).
  • Use an adaptive card - But I don't believe file upload is possible via this method.

Option 1 will work for you, but it is complex to build, so you will need significant expertise of Power Automate. I did a blog post on the subject of creating forms using a 3rd party tool hosted in Power Automate:

I don't think what you want to do is easily achievable. However if you do some investigation into adaptive cards, they will be able to do exactly what you need.

Thanks a lot @Paulie78 for the reply. Is the form creation part making it unachievable? Also, can we upload files through Adaptive Cards in our environment's dataverse? A model-driven app is used by users for tracking incidents and we need to make use of that dataverse.
