Flow condition creating duplicate records in SharePoint

My Flow objective is to update SharePoint List with items from Excel file stored in SharePoint folder.  Here is my Flow:

My issue is that I keep getting duplicate records added to my SPL. What's weird is that it is always the first few "new" records that are created.

For instance, my SPL is updated to add 5 new records. The first new record may have 5 duplicates, the second 3 duplicates, the third only one duplicate and fourth and fifth none.
