Hi,
I am trying to build a flow using Power Automate connected to Forms and to have an approval step.
My Form is set up in the following way:
Question 1:
Text Field - Managers Email
Question 2:
Option Field - Option 1/2/3
Question 3:
Upload Field - Document Review Upload
Question 4:
Rating Field - Out of 5
I am hoping to creating a flow that when the responder uploads a document in question 3, it needs to go to the email given in question 1 for approval first before it can go to the creator of the Form.
Any help will be appreciated.

i have just added that compose to see what comes in the outcome and it is the approved or reject message
Also here i have not added responders email as it is the same email which i have hard coded in the send email connector step.

I have managed to create this but it just goes to a standard approve/reject - not specific to Question 3 and cannot review the document.
Also @DrishM13 My question do you want to send an approval only when the document is uploaded ??
Hi @ViditGholam - the document upload is a required field so approval is always sent as a document is always uploaded
Hi @DrishM13 here is how you can do it create an automated flow.
Use these triggers as shown below don't use existing templates
Then follow this youtube link which will show you how you can attach the contents of your uploaded file in the attachment of your approval (he has attached the file to a sharepoint list but you can attach in your attachment)

Hi @DrishM13 I implemented (Create a Blank Automate Flow and use the same steps as I Did ) I haven't renamed any connection name so that it is simple for you to understand

Note - It is annoying to edit the one drive name it comes back again and again you will have to wipe it multiple times.

HI @ViditGholam - thank you so much - it works well and I like that you get an attachment of the file you have uploaded.
My only question is, can there be a step added at the end when an email notification is sent to the responders email address about the outcome?
I see the notification comes up in my Teams but not outlook.
I have tried to add "send an email" and I get the "To and Subject" but when I try to add in the body, there is nothing stating "outcome"

I added the "email address" of the requestor in the "start and wait for approval" step as it was only showing my email even when another person was asking for approval.

What I am looking for it after the document is approved or rejected, the responder gets an email at the end stating the outcome...something like this? But I can't seem to get a field that shows "outcome"

Hi @ViditGholam , I have a quick question about this flow.
Whenever someone else requests a document to be approved/rejected, the document does indeed go to the specified email "managers email in the form", however the response to the responder comes from my email and not the managers email address.
Is this because I am the owner of the form?
Is there any way around this?
I modified the flow to the picture below and wondering if this will solve it? Not sure if it should be 1 or 2?
