Flow failing to populate Word template from Forms

I'm trying to create a flow that accomplishes the following:


1. A user completes a Microsoft Form. (The form responses link to an Excel spreadsheet outside of this flow.)

2. The flow splits into two parts:

2a. The response is used to populate a Teams message.

2b. The response is used to populate a Microsoft word template, and that document is saved to a Sharepoint site.


2a is working fine; I cannot get 2b to work. The document is created and saved, but none of the info from the response is populated.




The template is .docx and saved on OneDrive. The fields where I want the Form responses to show in that document were created using the "Plain Text Content Control", and I've mapped them correctly in the "Populate a Microsoft Word template". When I open the saved document created by the last step, the content control fields are still present - I don't think it's just populating them with blank spaces.


The responses in the form are long text fields and may include line breaks.


I've reviewed the questions and solutions that showed when I search for "populate template". Can someone help me figure out what is missing or incorrect?

Do you create the Power Automate from a template?

If yes, the issue may be caused by the template is an old version, please try to structure it from blank.

Thanks for the response. No, I didn't create this from a template. 

Would it be worthwhile to attempt to recreate the entire thing from scratch?
