Flow for getting files from a form into a sharepoint folder

I want to create a form (doesn't have to be Microsoft Forms, could be excel, anything that can take in data entry + a FILE). Then, I want the form data + the FILE to go into a sharepoint folder.


Ex: My form: Name, Email, File

Sharepoint would receive the file and I would have some way of seeing in Sharepoint that this file came from [Name] [Email].


Problem is that Microsoft Forms doesn't seem to have a file upload input. Any suggestions?

FYI - adding an attachment to Microsoft Forms is a highly requested feature and "in the works" according to Microsoft

With that said, why wouldn't you just utilize a SharePoint List Form? You could even create a PowerApp to utilize the List Form and provide a simplified form GUI to your users (documentation).
