I am trying to build a flow that checks the CDS Customer Voice Invite entity (table) daily and send a reminder email with a link to the original survey sent x number of days after the original survey was sent. My flow passes the flow check but fails when it finds a record that meets the criteria.
The original survey is triggered by a PowerApp button connected to a CDS entity (table) I created named Project Follow-Ups (PFU). The Project Contact in the PFU table is a lookup to Contacts and the send to email address, FName and LName are calculated fields pulling from the CDS Contacts table based on the lookup Project Contact field.
I have posted screen prints of the Flow in two versions. The first one is trying to get the contact details and email address from the CDS Contacts table. The second one is trying to get the contact details and email address from the CDS entity custom PFU table. Both flows are in the same place. I have also included the error message for each flow.

What we discussed is as follows:
1. Get the Respondent full name.
2. Filter the records (Project Follow up) using First name and Last name to get the user email.
3. A new lookup is needed under Project Follow up entity to identify which survey has created against such as such project and contact.

Not sure how you filtering the List Activity Parties.
You could add an IF condition after the above step to determine which party type is whether its contact or project follow up.
Then under the Yes condition you can use the below step

I don't think your suggestion solves the problem. If you look at the beginning I am only selecting from the Customer Voice survey invite table to identify surveys that are open after x number of days from the date created. The original surveys were sent from the custom CDS table named Project Follow-Ups. The Project Contact is a lookup from Contacts and the send to email address is a calculated field to get the email address from the related Contacts record in the Contacts table. I only tried the flow using Contacts because the flow with Project Follow-Ups was causing an error. I am wondering if the problem is that I am working with a custom entity with a lookup for the Project Contact. The bottom line is this flow should: Daily check for [Customer Voice survey invite] records to find any that meet the "Date Created" plus x days criteria. If there are matching records, find the FName and Project Contact email address from the [Project Follow-Ups] table used for the original survey and then send the reminder email.

I am sending the survey from a custom entity named Project Follow-Ups which contains the Project Contact, FName, LName, and email address. I only included an effort to pull that information from Contacts (the Lookup for Project Contact) to show that wasn't working either. The information I need is in the custom table Project Follow-Ups based on an open Customer Voice survey invite record that needs to go back and get the name and email address for a reminder email.
