Get Item and create an Email when a planner task is assigned.

Ok, so here's a fun one. We have a sharepoint form that, when filled out, creates a Planner Task and updates the description to include a link to the SharePoint item. When they are initially created, the Planner Item is unassigned. What I want to be able to do is, when the item is assigned/reassigned in planner, create an email from a shared inbox with both the link to the task and the details from the original sharepoint item in a table. So far I've managed to create an email that sends every item from the list, however I want it to just focus on the one that has been assigned
