Import Statement from MS Forms likert into Excel if a specific answer is chosen

We are build a flow/Power Automate that will import answers from a Form.  The target result is to have one column that will show all of the non compliant results.

When trying to import we can only get the answer rather than the statement.

There are 36 statements with the choice of YES, NO, N/A if NO is chosen then it isn't compliant therefore we need to know the answer.  This could be the statement or a reference number 1.0, 1.1, 1.2 1.3 etc
