Issues with All Approvers Must Approve Flow

Looking for assistance in fixing this error. The flow seems to work well with First to Respond. PA continues to add in Apply to Each below condition line.

he execution of template action 'Apply_to_each' failed: the result of the evaluation of 'foreach' expression '@{outputs('Start_and_wait_for_an_approval')?['body/responses']}@{items('Apply_to_each_3')?['responder/displayName']}' is of type 'String'. The result must be a valid array.

Could you please share more details about your scenario?

When a cloud flow with the Start and wait for an approval action is configured with Approve/Reject - Everyone must approve, it waits until all Assigned to approve or at least one Assigned to rejects the approval request.

So actually the outcome the approval action is only a line of text, no matter how many approvers are there and what their responses are.
