MS Flow using MS Form data - Multiple Fields

I have a flow that is routing MS form data gathering check information to various departments depending on criteria. This has worked fine, but now the requirements have changed a little. There are four fields that needed to be duplicated to allow for multiple checks to be entered. The rest of the data is the exact same. The two portions of the flow is to route an email and to create a list item in SharePoint. In the case of sending out an e-mail I just created a table and put in the multiple fields.

 

I cannot figure out how to create 2-5 different items, changing just those four fields. Is that even possible?

Can you provide a little more detail on your requirement - I don't have a clear picture of what your are trying to accomplish.

My Form looks similar to this:

1. Associates Name:

2. Reference Number:

3. How many checks are you entering?: (Options of 1-5)

4. Check Number:

5. Check Amount:

6. Check Date:

7. Name:

(4-7 are then duplicated five times depending on option 3)

 

So my output is five fields marked as Check Number, and so on.

All of the other data is exactly the same.

 

The first action is to compile an e-mail to the appropriate department. And the duplicates are fine because I just have the fields all organized in an HTML table.

 

But where I am getting stumped is how to setup a "Apply to Each" action when adding the records into my SharePoint list. Each row should have identical information minus questions 4-7, those would be unique to the duplicated data. 

 

I know I am not doing a great job explaining this. I hope this makes sense.
