Mass/Bulk Approvals and list update not working as expected

I am in the process of creating a way to mass approve items in a list but am having some difficulty and hopefully some very intelligent people here can help me solve it.
I have a list "Access Requests" that hold various information created by users and this information needs to be approved Bi-Yearly by each employee's manager on whether its valid or not.
1. Scheduled workflow runs every six months - Checks all the data entries in the "Attestation Required" field in the Access Requests Lists to see each one is set to "Yes" and if it is, change it to "No". This then tells the "When an Item is Modified" trigger to work.
For each item in the list where the Attestation field was "Yes" but was changed to "No", should send an approval to the manager asking if the item is still relevant and if it is, then change the "Attestation Approval" field back to "Yes".
What is happening is that I am getting the correct count of approvals, but it is sending the same approval for one record and not the entire list of items. Also, when it is updating the log list it is updating the same record data for each item.
