Microsoft Flow To Form Running Inconsistently

I have created flow from a Microsoft Form to a SharePoint List. There are multiple branching activities to move the responses into the List and then score the responses in SharePoint.

The flow works correctly some of the time, but others it shows the error "ActionFailed. An action failed. No dependent actions succeeded." This appears after I have Form flowing to a SharePoint List and Updating An Item to score the results.

I cannot find a consistent pattern for why it does not work on those occasions. Any help would be appreciated!

No, it has failed when office relocation value is other text as well in the branching. 

The Form text moves into a SharePoint list column for single line of text and that informations is coverted to a score of 1, 2, or 3.

Could you please share more details about the office relocation score column? What is the type of the office relocation score column, is it Number type?

Could you please share a screenshot of the run details of the Update item action when it is run failed?

Certainly! The Office Relocation Score branch is designed to identify whether the answer is "Yes" or "No" to a Form question and fill that in SharePoint list column for a single line of text. If it is "Yes," another SharePoint list column should populate with the number "1." The Office Relocation Score column type is a number.
