Moving attachment and other responses from MS Forms to Outlook for Approval before moving it to SharePoint List

I once got this error "AttachmentContentNotValidBase64String"

I am moving form responses along with attachment to SP list through automate.

After user submits responses on MS form- user gets a confirmation email. Then- i update data on SP list. After that an email for approval goes out to manager

Once the approval/ rejection is done- the email is sent/ back to the user- the SP list is updated with approval status.

The error is that i want the attachment that user attached initially in the form to flow through Outlook for approval and then finally to SP list. But even before it reaches the approver- it throws this error.
