My flow keeps cancelling even though the condition is false

I have a flow that is getting cancelled even though the condition is false. For reference: this flow connects a Microsoft form to a plan on our team's Planner, creates separate tasks from one submission depending on the type of work requested (4 different types of work, each put into their specific bucket based on the form's responses), and adds a row to a spreadsheet with the basic information from the form.

For this flow, there are four work requests: RI, CS, FS, and OSD. If I only request RI, the flow works as expected by creating the planner task and adding to the spreadsheet, but when it reaches the second condition to check if CS has been requested it cancels. This only happens if I only request RI; if I request RI, skip CS, but request FS and OSD, it works; if I don't request RI, but request all other three it works. The only issue is when you only request RI, it just cancels the flow. Please see screenshots below.
