Power Automate Trouble

Hello,

I am trying to build a flow from a form submission. Once a specific form is submitted, I want to be emailed the notification and have the information uploaded to Sharepoint. 

The only step in the process that is troublesome is when I put "Apply to Each." It asks for me to "select an output from previous steps" and I am only given a few options in this step. I have used every option offered but the flow still fails at this step. Why is that? How can I fix this?

You don't need the Apply to each.  The flow will be triggered each time a form is submitted.  Just use the Response ID to retrieve the response details.  You are probably looking at an old Blog post.  When the form actions for flow were first introduced they triggered on a collection of responses.  That is no longer the case. They now trigger for each form submission.
