Product Feedback Flow (Forms + Planner + Teams)

Hi there,

I am trying to build a flow that will let a user fill out a very brief (3 question) form, have a planner task created, and have the Notes section of that planner task populate with the three questions/answers from the form. I was able to get one create that creates a task where I want it, but I cannot figure out how to get the Notes section filled out with the Q/As

Can you provide some more specifics for how I can make this work? I am attaching a screenshot of the form as well as the current flow. I want each of the three questions to be added to the description.
