Questionnaire that once filled populates a form and gets sent for approval

I'm looking to make a request form process easier.

 

currently staff have to print out fill in, and return a paper form for approval

 

I want to make a flow with power automate

 

The idea is staff scan a QR code or follow a link or access a tablet, fill in details on a form or questionaire and select dates, which once finished pre populates a copy of the current leave form an sends it to me as an email. for approval.

 

I have to keep the template the same as the form is used with payroll.

 

I'm struggling to find an avenue for this help?

Hi @Noobflow 

 

According to your requirements I have an idea, first create a flow that triggers every time a new answer is added in the form, extract the answers and send an approval to your email with the answers, then if you approve, populate a Word template with the answers (This word template should be the copy of the current leave form).

 

For example, something like this.

It will vary depending of what you need, also you decide what to do if you don't approve it.
