Send email based off form field value

Hello,

 

I am stuck on this flow, am probably missing a couple of steps and could really use the communities help:

 

I have a basic form and one field is called "Site Access Code".  I need to translate this assigned code to an email address in order to send the response to the correct recipient.

 

We have to track individuals visiting construction sites.  Each construction site is assigned it's own "Site Access Code".  When the visitor scans the QR code and fills out the form they enter the code posted at the site.  For example, 2284.  

 

I cannot figure out that after I convert the responses from HTML to PDF, how do I email the PDF to the correct recipient based off of the code.

How we could know the information you provided below:

Site Access Code - 2284 = jsmith@company.com (recipient of the form)

Site Access Code - 1978 = ttester@company.com (recipient of the form)

 

You need to have a list with the Code column and the email address column, so we could find the email address of the code from the list when a response is submitted.

Hi @Armorbee ,

 

Could you please explain more details about your requirement?

Do you want to send an email with pdf attachment contains the from response?

And do you want to send the email to the person the Site Access Code lead to? As your example, when the code is 2284, what would be the email address? do you have a list to check for the email address for a special code?

 

Please share more details so we could better understand the issue and provide a proper workaround for you.

Thank you for the responses.  I included the flow I currently have at the bottom of this reply.,

 

Here's the breakdown on how the form is intended to work start to finish:

 

1.  User scans QR code at jobsite to fill out survey form.  On a posted sheet outside the trailer is a Site Access Code number, which translates to the job site they are completing the survey on.

 

2. One of the questions on the form is to enter the Site Access Code displayed on the sheet.

 

3.  Once the user submits the survey the results are converted to a PDF, The PDF saved in a one drive folder, and then the pdf is  emailed to the email address that matches the site access number.

For example:

 

Site Access Code - 2284 = jsmith@company.com (recipient of the form)

Site Access Code - 1978 = ttester@company.com (recipient of the form)

 

I can get the HTML converted to PDF, the PDF saved, but I cannot get the workflow to lookup the site access code number and populate the TO field in the email based on the value.  I thought I could use a Switch command, but it fails every time.

What is the purpose of the compose?  With my current workflow I can create the PDF send it as an attachment, I just can't make the logic determine who to send it to.  Where does the compose go?

@Armorbee  the compose is becouse I understhad that you need to use the value when a user sends a response.

Thank you for that info.  I am going to have lookup how to use Compose. 

That's what I am trying to figure out.  Where do I store those values.  I thought I could use SWITCH and say

 

if Site access code = 2284 then (populate TO with jsmith@company.com)

 

I know the values, I just don't know where to store them.  

Your comment about how I am looking up those values for email addresses made me realize that I wasn't storing those values anywhere.  What I did was create a new excel table, populated the site access code with the correct email address and wrote that to a variable that I populate in the To field of the email.
