Simple Notifications for List Additions Don't Work Properly

I am trying to make a very simple Flow to post a message in Teams whenever a new item is added to a SharePoint List, with some content from that item included in the message. I have a Choice column that I want to use to format the message in teams (the Choices are "Planned" and "Unplanned"), so that Planned items can be posted with different verbiage and colors than Unplanned items. I can get a message to post every time a new item is added to the list, but it won't display the correct information. I've tried a couple different routes, but I always just end up with content from the first item added to the list, or sometimes just a message for every item on the list, instead of the newly added item. Is there something simple I'm missing, here? If I create a flow without the Condition, it works fine, but I can't format it as effectively.
