Trigger from one a specific form to create ADO Work item

Hello Everyone,

I have a question around creating a flow with dual triggers, specifically in Logic Apps. Our goal is to create feedback ‘Like & Dislike’ buttons at the bottom of our ADO Wiki pages.

So, someone clicks either Like or Dislike button to bring them to the appropriate Microsoft Form:

  1. Like to one Form
  2. Dislike to another

Once they land on the appropriate Form, we want the flow to have an understand of which form was triggered and branch off from there to create an Azure DevOps work item.

I figured starting with the Microsoft Form Trigger ‘When a new response is submitted’ is triggered, then we can branch off with a Yes or No flow. However, I cannot seem to figure this out and maybe I am on the wrong track for implementing this.

Any suggestions?
