Unwanted duplicate row added when 'adding records to Excel'

My flow works successfully.  However, whenever a record is added initially into Excel, it is adding a duplicate row.

I cannot figure out why i'm creating 2 approval requests instead of just one.

I'm adding my flow for consideration.

I don't see any nested loops. Nor do i have any 'Apply to each' at this point of the flow.

My assumption is there is a second flow that is also being triggered by that form response. Maybe in a different Environment or the same Environment.

OMG!! I didn't even think about that!  Yes! I'm sure you're right. i've turned off the test flow using that same form.
