It looks like it only took the Segment not the rest
I have searched and seen that this question has been asked before, however I am new at using Flow and need some help.
I have a list called CounterMeasures that the following columns:
I have another list called Heatmap which has the same fields, where I am having the issue is that I need to have the first three columns match exactly in both lists before you update the Actual and Target to the Heatmap list.
So they basically update/create an item and if the Segment, Modality and KPI match in the Heatmap list it copies over the content in the Actual and Target to the Heatmap.
I dont know how to ensure that match works and i need to figure out what I am missing. I would really appreciate someones help.

@Ruenells as we saw together we had some troubles with the OData , because the column have some spaces, and i don't know why it isn't working, but we had workaround this by using an condiction inside apply to each to ensure that the values will be the same.

You would Add a condition to a flow, specifically an and condition with multiple lines. See the Use expressions in conditions to check multiple values documentation, and this example:
Be sure that both of the SharePoint Lists have the desired columns in both of them added to their default List views. In the Update item step, you'd pass the non-matching columns.

If you could provide an expanded screenshot of your Flow and steps, and of any detailed error messages you're receiving we could likely better assist you.

The flow that you need must follow this steps:
1 - Trigger when the List 1 item was modified
2 - Get itens from list 2 filtering by the fields of list 1(with OData), if the columns has the names that you post paste the expression below inside Filter Query
((Modality eq '@{triggerBody()?['Modality']}') and ((KPI eq '@{triggerBody()?['KPI']}') and (Segment eq '@{triggerBody()?['Segment']}')))
3 - Update all itens found

You use the OData filter?
If yes, you don't need the condition, otherside your 'Get Itens' will bring all itens of Heatmap list, this will undermine your performance and you will have troubles when your list has more than 5k itens, but works too

In your screen shot your Get Items is taking from List 1 is that correct shouldnt it be taking from List 2?
I am trying but when I use the expression you provided and the Dynamic Content shows up it is only pulling from List 1 and the fields it is showing is saying Modality Value not Modality so it is failing. However, I know those fields that I put before are correct. And am I supposed to be pulling the data from List 1 or List 2?

You can put the expression I've provided, remove the dynamic fields and put the correct fields, one point of attention is the field names in List2, if they aren't the same as the ones I put you need to change, you could post here a print from the execution history?

I am getting this error when I run this

The name of column in Heatmap isn't Modality, open the List Configuration and open the column, in the url you will find the correct name like this:

It's right, you will only see the field from List2 below the action 'Get Itens'

Ok fixed that and it ran successfully however it didnt update anything in list 2.

Remove the Segment ant put Segment Value.
One thing that can make visualization and change easier is to create the Compose action and put the value in there, and then at the place of the fields you would put the output of Compose action, like below,:

I am confused as to why you want me to replace the field that it pulled correctly it was the KPI and Modality that didnt get queried?
