Using SharePoint with Forms and Power Automate

I have a need and I'm not sure exactly how to create the flow. I have a SharePoint list that holds "intake" items. On regular intervals (1 month, 2 months, etc) after list item creation date, I need to send a form out to the submitter to gather updated info. I need to then capture the form responses, which includes ability to add attachments, and write those responses back to the corresponding SharePoint item.

I have the form created, but not really sure where to go from there and not sure how to tie it to the right SP item ID.

hi @kriggo15  you could use a tick, I asume that you are going to send the link forms by email, you you can send in the body the ID of the sharepoint row, then ask to the user to fill that ID in a form field. Of course the user could make a miskate, but is the only way because see forms and sharepoint are repareted platforms.
