I had a Flow set up that was triggered from within a Model-Driven app I made. It was a sort of test Flow - it just updated the Approval Status of the record I was currently on. Recently I noticed it had stopped working, and when I went to the Flow setup, I found that the 'Environment' value, which was previously my specified custom environment, had been changed to 'Default'. and it doesn't even give me the option of selecting my other environments, none of them appear.
When I select another step ('When a record is created/deleted/updated') all of my environments appear fine. It's just 'selected' where I have this issue. Has anyone else had this problem?

The "Default" environment option in this trigger is basically referring to the environment on which you are creating the flow. (Basically, this is not the actual default environment of the tenant). As long as you have the Model driven app and the flow in the same environment, you should be able to see the flow on that particular entity that you selected in the trigger.
