update item successful but no action

So created a flow this week to simply update an item's title once it has been created and to update a field to state the title has been updated.

It was working perfectly until yesterday when I demo to my boss. the flow states successful run and I can trace the events through to the update list item which has a green tick and the outputs are all matching what I would expect. unfortunately, you find the item in the list and the item's title doesn't get updated.

There is no pattern to this and it works most of the time. If I re-run the flow then it succeeds and changes the field which it shouldn't because although the title doesn't update originally the field to say it's updated does.

I'm at a loss because I could create a duplicate flow to double-check this, but surely if the first can falsely succeed then so can the second

Hi @JoePike

Is this Dynamics 365 records you trying to update? Check the audit history?

Thanks

UPDATE: I have just done some more testing with another similar flow. except this one is triggered on powerapps button press.

it updates a field to budget check which kicks off another flow, and also updates the approval sent field like the flow discussed above

trigger - tick

get item - tick

condition met -  true, tick

update item - tick

update item output as expected - yes all looks fine

budget check field updated - No

approval sent updated - yes

The only difference in behaviour is that this time when I re-run the flow the condition is not met because the approval sent field is updated. Really thought I was coming to terms with Flow/PowerAutomate, but now it all seems a bit wishy-washy and runs on magic and fairy dust.

Hi @abm, Unfortunately not this is just a bog-standard SharePoint list item

Hi @JoePike

Delete the current trigger and add a manual trigger. Save the flow.

Run one record manually to update the title.

Once it ran successfully then edit the flow and add the old trigger again.

Let me know the results.

Thanks

Hi @abm , Sorry I have been busy with projects and trying to meet deadlines

I managed to do some more debugging and because the title was a required field it was being set to the default form value, which was setting this after the flow had run which gave the illusion that the flow was running and not changing anything
