How to put multiple answers in a send email action?

Hi all,

I managed to have PVA send out an email but with one single question and answer.
So right now I have:
Question: Please state your request
User: States request
And an email is being sent with the request in the body of the email.

But I want the user to be able to ask two questions. One is 'please mention your location', and, 'please state your request'
So these two values should go to the email. The location in the Subject line and the Request in the body.

How can I manage to do that?
