I think there is no way to directly backup a custom list form in PowerApps. Yet you could recover the previous version:
I just tested below steps, after you accidently deleted one SharePoint list, you could recover it from the Recycle Bin, and the related custom list form would also be recovered.
1. Go to your site and click on settings as shown below.
2. On the settings page select “Recycle Bin” under the Site Collection Administration section.
3. When you click on the “Recycle bin”, you will be navigated to the recycle bin page as shown below. There you can select the list or library to be restored.
4. Then click on the “Restore Selection” option to restore the list. To confirm the restore click the OK button on the pop-up.

This will work fine unless the admin empties the site recycle bin ( 2 stage delete)
So yes, restoring the list will restore the powerapps form - I tested and that does work... Thx for the detailed instruction! 🙂
