Do you have done any upgrade on PowerApps ? Since yesterday it's impossible to add a new column in Excel Online Office 365.
I have took a model (Site Inspection) and then I have modified the template (Excel file "Data" in One Drive).
Then, when I try to add a new entry, I have the following message in attachment.

The issue might be caused by the data source connection initialization.
Please take a try with the steps mentioned by vivekb in the following thread:
Copy the created Data.xlsx file into another location, after that, rebuild the connection between PowerApps and the related Excel table.
