These screen shots are all from SharePoint. Did you do the one where you clicked on your list and did "Customize forms"?
If so, you need to click on PowerApps/Customize forms and look there for what the issue could be.
Things I would start with are:
Form - DataSource - Should be your sharepoint list
Form - OnSuccess - ResetForm(FormName);RequestHide()
Form - DefaultMode - Edit
Form - Items (something similar to) - If(IsBlank(SharePointIntegration.Selected) || IsEmpty(SharePointIntegration.Selected),First([@SPList]),SharePointIntegration.Selected)

In SharePoint list, if no items are seleted, then you could create a new item with PowerApps Form mode in NewForm mode like this:
Just select "+" and custom form would open with a new form.
Only when you select one single item, then Edit button would apprear, after selecting it, Form would be open in edit form mode:
Also, please make sure that you are using the PowerApps custom form by navigating to List settings > General settings > Form settings.
Yes it is using the custom form, when I create a New record or Edit a old record from the Home Page where I am displaying the List it uses the New form. However when I go under Lists and open the list and select New or a recored to Edit it does nothing.
