Add multiple values for multiple date in Canvas App

I have a canvas app

For a date (Say 24 September) I should be able to enter values in General and Report Writing Field

For another date (say 18 July) I should be able to enter values in some fields

And when I click on 24 September the entered values should populate

Similarly when I click on  18 June , the netered value should populate,

 

Please let me know how to achieve the same

i tried using Patch function but I was not able to populate values

Notify("Time Entry added successfully");
Reset(datevalue);
Reset(A1);
Reset(A2);
Reset(A3);
Reset(A4);
Reset(A5);

 

Lookup function in each field :

LookUp(TimeEntryCases, ddi_caseidentification = [@ModelDrivenFormIntegration].Item.CaseID && Date = datevalue.Text, ddi_administrative)

You need to Patch conditionally - only enter the details if they are not Blank.
