Autopopulate two text field from selected dropdown

What is the best approach to display value in two text box screen shot showing below when the user selected a value from the dropdown.

So when they select a value from "Home Center" dropdown It should auto populate "Supervisor", "Regional Manager"

 

SharePoint List: Centers

Title: is the Center Name

Supervisor: Supervisor Name

Regional Manager: Regional Name

On the dropdown item list put: Centers.'Center Name'  (remove the single quotes if there is no space : Centers.CenterName)

Then in the default of the Supervisor textbox: LookUp(Centers,'Center Name'=dropdown.Selected.Value, Supervisor)

Then in the default of the Regional Manager textbox: LookUp(Centers,'Center Name'=dropdown.Selected.Value, 'Regional Manager')

To make a good suggestion I would need to know what SP column types Supervisor and Regional Manager are (e.g. single line text, Person, Choices...).  Please let me know. 😄

Center SharePoint list has the following Column and Type

Column                        Type

Supervisor                    Person or Group
Regional Manager       Person or Group
Center                          Lookup
Title                              Single line of text

I was able to figure it out with the help of @eka24  

 

I use the following formulas

LookUp(Centers, Title = DataCardValue10.Selected.Title, Supervisor) 
LookUp(Centers, Title = DataCardValue10.Selected.Title, 'Regional Manager')

Now when a user select the Combo box value it auto populate "Supervisor" & "Regional Manager"
