I have created what I thought was a simple request form for another group, mimicing an existing process. It is housed in SharePoint, and Choices are supplied by Lists. After consultation, one of fields changed from Single line of text to Lookup. I dropped the field and re-added it to the form, but the Choices are numbers rather than the items from the SharePoint list.
Where should I look for the error?

Unfortunately, this error had to do with the number of Lookup columns in the list. I had 13, and the limit is 12. I deleted one, but now I am getting the "No item to display error." Seems like it is time to rebuild the form.
