Combo Box selected records to display in a Form

Hi,

My requirement is to display the selected records, from a combo box in Datacard on a Form.

The Combo box displays the records in a gallery

When the required contacts have been selected I'm hoping they can then be displayed in a Form

Then when additional information is added, these are then added to a SP list.
Any assistance gratefully accepted
