Connect Sharepoint to App Created from Scratch

Hello,

 

I created an app from scratch.  I was able to import the Sharepoint list as a data source but I am not sure how to link my controls to my SP columns.  Can anyone please offer me some assistance?  Thank you!

Thanks Mabel! I was able to figure it out by add the Patch formula to the 'OnSelect' event of my Submit button.
