We created a custom object with the same name as an OOB entity, renamed the OOB entity, but Flow doesn't differentiate between the two. Notice the duplicate "Work Orders" at the bottom of the list. Why doesn't Flow represent the updated names in the entities list?

Unfortunately, it may have cached references within it. Perhaps try the copy/paste to clipboard functionality within Power Automate to replace it, or can you create the Action again (perhaps above it in the builder, and then delete the older one)?

Ensure you have published the customizations within CDS.
There may be a slight delay in other things, such as Power Automate, picking up the display changes. Can you confirm if you create a new Flow, after the publish, if the changes are represented?
Changes were published a while back. This is only seems to happen with the first action being defined. Later references to CDS objects work properly.
