Get Edit Form Record related to ModelDrivenFormIntegration

I apologize for the multiple messages today. I've scrolled through docs and other posts for the last few hours and haven't found any similar issues. If there are others who have encountered similar issues, please direct me that way!

I’m trying to get an edit form to display a specific record that will then be edited using the Patch function. However, I’ve been unable to get this record to display.

Am I able to get the entire form to display a single record (associated with my ModelDrivenFormIntegration), that a user can then edit? If so, how?

I’ve tried using the LookUp Function on the Items property with the following formulas where “Active Enrollment Career Majors” is a collection of all Active records in the form’s Entity:

LookUp(ActiveEnrollmentCareerMajor, 'ActiveEnrollmentCareerMajor'= First(ModelDrivenFormIntegration.Item.'Enrollment Career Majors')

LookUp(ActiveEnrollmentCareerMajor, 'ActiveEnrollmentCareerMajor'= GUID(First(ModelDrivenFormIntegraton.Data)ItemID))

What am I missing here?

A note: While there will only ever be one active Enrollment Career Major associated with the ModelDrivenFormIntegration, there may be several others in different status’s.
