What should be my approach if I have to put this (see attachment) in my Canvas App? I am using SharePoint list as data source. How should I create and patch this to one or more lists. Please help!
I have written an article on how to do this. I’ll link it below. Suggest you build the app in my article exactly as shown. Then once you have it working start tweaking.

@mdevaney Hey Matt!! Thanks for responding. I follow you twitter and I have read this. The grid is bit different here. If you observe, I have columns or fields both vertically and horizontally. (PAN No, TDS,....) is one set of columns which are vertical, and (Applicable, Proof of Registration,....) is another set of columns horizontally.

I am wondering about your grid. Why are there many options where the user can select Yes or N/A. Is it actually possible for the user to select these? Or are they always Yes?
