Is it possible to add a survey to a SharePoint web page?

Hi All,

Is there a way I can add a survey to a SharePoint webpage so all the users in the company can participate? We want to ask a question once a week where the user can make a selection.

Example Survey Question:

If you could pick a day to work from home what day would it be?

1) Monday

2) Tuesday

3) Wednesday

4) Thursday

5) Friday

You could link to an MS Form and then use a Flow to add a record to a SharePoint list.
