I have the image I want in local collection. Before I write these back to Sharepoint I want to offer the user the option to sketch on top of the photograph and then save this back locally. Is this possible?

I've been thinking about how this could be done just in PowerApps. I personally don't think it can be done.
My suggestion/solution would be to create an azure function script that takes in both images - the base image (The image you are going to sketch over) and sketch image (perhaps using the pen input), which then merges them, and returns the the merged image.
OR
Find an API that has open source to then put into azure functions rather than you writing the logic. Using Azure function apps will let you easily connect the api to your powerapps.
Would love to hear if anyone else has a simpler solution.

But to answer your question - Yes. I do think it is possible to do such a thing, it just may not be so easy.

No, I don't think currently it is available to achieve that in PowerApps.
Maybe staff @CarlosFigueira could share some additional information.
I think a better approach should be to add a backgroud property for the Pen Input to allow add a custom image,and then make it possible to sketch on the image directly.
