Issue Loading Information Using LookUp

I have a form where I am loading data from a SharePoint list (load form saved progress). I am creating variables using the stored data in the SharePoint list and assigning the variables as default values in the fields. This is working well for comboboxes:

Set(TemplateSoldTo, [LookUp(TEST_FormInProgress, UPN = UserID.Text, SoldTo)]); 

 

However, when I am trying to load into dateboxes or textboxes I get issues that look like the following:

The code looks the same:

Set(TemplateShipPO, [LookUp(TEST_FormInProgress, UPN = UserID.Text, ShipPO)]);
Set(TemplateSurgeryDate1, [LookUp(TEST_FormInProgress, UPN = UserID.Text, SurgeryDate1)]);

I am using 'Date/Time' set to only store date datatype for the Date field and 'Multiple Lines of Text' for the text field in the SharePoint list.

I am not sure how to get this to work; any help would be appreciated.

Can you update the expressions as:
 
Set(TemplateShipPO, LookUp(TEST_FormInProgress, UPN = UserID.Text, ShipPO));
Set(TemplateSurgeryDate1, LookUp(TEST_FormInProgress, UPN = UserID.Text, SurgeryDate1));
 
The issue here is, combobox supports table schema value but TextBox/DatePicker doesn't. Thats why you need to pass a  single value as the default values.
