Hi, I do not know why this automatically changes the UI of approval for my Users, and now it's for me as well. However this was not the problem before, It was hardly once or twice I have seen such UI in approvals flow getting a broken, but used to get resolved on its own after few hours, but now since a week I m getting the same error and it's redirecting me to the other link where my users are prompted to Login into PowerAutomate account, which is worst, and annoying for them.. Many Complaints every day.
Users are not able to approve using the email interface provided by Approvals.

Flow overview looks no problem and it is OK if you use newest action of Approval connector.
As additional point, visualization behavior of adaptive card depends on Outlook clients. So if you have changed Outlook client, you will see different behavior.
e.g.
- OWA
- Desktop Office 365 Outlook
